The ad promising money back in your Social Security check is not, in itself, a scam. What it is describing is the Part B premium reduction — the “give-back” — a genuine Medicare Advantage benefit that carriers report to CMS every year. In Fleming County, Kentucky, 23 Medicare Advantage plans include a Part B premium reduction for 2026, and the largest reduction available in the county is $184.70 per month.
How the Part B premium reduction actually works
The Part B premium reduction lowers the amount you are responsible for paying toward your Medicare Part B premium. How it shows up depends on how you pay that premium: if it’s deducted from a Social Security or Railroad Retirement payment, the reduction may appear as a smaller deduction (a larger deposit); if you’re billed directly by Medicare, it may appear as a credit or reduction on your premium bill. Two important qualifications: processing can take several months to begin, and if Medicaid or another assistance program already pays part of your Part B premium, the reduction may apply only to the portion you pay yourself. You remain responsible for any remaining Medicare premiums.

Who the give-back actually reaches in Fleming County
A Part B reduction only helps if you are the one actually paying the Part B premium, which is why those numbers matter more than the headline amount. Roughly 825 people in Fleming County qualify for Medicaid assistance as well as Medicare, and when a state program already covers part or all of a Part B premium there is less of it left for a plan to reduce. If that is your situation, the give-back is usually the wrong thing to be shopping on. The other 1,301 residents are on Original Medicare, where a give-back does not apply at all.
The network question in Fleming County
A reduction does not change the part of the plan that decides your care: the network. The CMS hospital registry lists 1 Medicare-registered hospital serving Fleming County and it runs emergency services: Fleming County Hospital (Flemingsburg). Before a reduction is worth anything to you, check that the plan offering it covers the hospital and doctors you would actually use.
Most Fleming County residents live in or around Ewing, with Flemingsburg making up much of the rest. Provider networks are drawn plan by plan, not county by county, so two neighbors can hold plans with the same reduction and completely different access to the same doctor.
How to weigh a premium reduction
A larger Part B reduction does not necessarily make a plan the best overall choice. Plans differ in their provider networks, prescription drug coverage and formularies, copayments, in-network maximum out-of-pocket limits, and additional benefits — and those differences can matter far more over a year than the monthly reduction. Before choosing any plan, compare its network against your doctors, its formulary against your prescription list, and its total expected costs against the alternatives available in Fleming County.
Put a number on that trade-off with Fleming County’s own plans. Across the county’s Medicare Advantage plans the in-network out-of-pocket maximum runs from $4,000 to $9,250 a year (median $7,500). The gap between the cheapest and most expensive of those is far larger than a year of give-back at $184.70 a month, which is the whole reason the reduction should be the last thing you compare rather than the first.
